Warning... If you have Pop-ups disabled the release form will not be displayed. If this is the case simply print the blank release form below and mail it to us with your information.
When you press the "Submit" button the information on this form will be electronically added to the DBK database. In compliance with the COPPA Act of 1998, if you are younger than 13 the profile will not be displayed until we recieve the signed Profile release form from your parents via fax or mail. This will be displayed on screen so you can print it out.
To include pictures with your profile, you should E-mail these to Webmaster@DirtBikeKids.com. Pictures should be JPG, GIF or TIF format, at 72 DPI, and approximatly 200 X 200 DPI in size. Make sure you tell us the name of the Rider in the e-mail!
Profiles will be deleted from the database after 2 weeks if the parental consent form is not received for children younger than 13 years old.
